Real Nappy Pack Application Form

To qualify for a free nappy pack you must:

• - live in West Berkshire and provide proof of address
• - have a child under 18 months old at the time of your application
• - be able to provide a copy of your child's birth certificate or MAT B1

We currently deliver free of charge to a local library within West Berkshire and will email you when your pack is ready to collect.
