A pass for those who are physically disabled, have a learning difficulty or would be refused a driving licence on medical grounds.

Proof of eligibility required
1. To make an application you will need to meet the eligibility requirements and also be required to upload the following:
  
  A passport standard photograph (for more information, please read the GOV.UK page on rules for passport photos).
  
  Proof of disability
  - PIP
  - Higher Rate Mobility
  - Medical Forms
  Proof of residence (Less than one year old)
  - Council Tax Bill
  - Bank Statement
  - Utility Bill
  - Driving Licence
  Proof of age (Minimum age of 5)
  - Passport
  - Birth Certificate (if name unchanged)
  - Driving Licence
  - NHS Medical Card
  Please note: a driving licence can be used as proof of age or proof of residence, but not both.
