Renewing your National Off-Peak Bus Pass

This is a form to request a renewal of your National Off-Peak Bus Pass.

If you have used your pass on a bus service in England in the 18 months prior to expiry and your details have not changed, a new bus pass will be sent out a couple of weeks before your pass expires. So you should not need to complete this form.

You can request a renewal via this form if one (or more) apply:

• - If you have not used your bus pass in the 18 months prior to its expiry
• - Your pass has expired and you have not received a new one
• - Your address has changed (you will be asked to provide proof of this change)
• - You have used your pass primarily on bus services provided by Transport for London (TFL)
• - The bus service you use does not usually have a ticket machine

Most disabled bus pass applications will be renewed without any need for further verification, however if further evidence is required you will be notified prior to the expiry date.
