My Family Plan - Information for Professionals
This page is for professionals seeking further information and support on My Family Plans.
My Family Plan is a tool to identify and agree support for a family as soon as a problem emerges, at any point in a child's life.
Families work alongside agencies to help create their plan and identify which early help services they would benefit from.
Alongside the lead professional, the family, use the tool to explore what are the presenting difficulties and identify areas for change.
The My Family Plan uses a strengths and solution based approach to identifying needs, and incorporates positive factors in the family's life that may reduce the likely risk of further family dysfunction/ distress.
The family, together with the supporting agencies, use the tool to understand any challenges/issues. They then agree together what actions are required, by who and when, which produces the My Family Plan.
The My Family Plan can be used to track and assess the impact of the support offered on the family. Reviewing in this way provides an evidence base for additional support/intervention or, equally, a reduction of support/intervention. This is key to providing the 'right support at the right time'.
If you decide to start a My Family Plan for a family, you will then be the Lead Professional for the family. The Lead Professional role can change, dependent on which agency is best suited to take on that role. This can be decided and reviewed within My Family Plan review meetings.
