Short Breaks Services Feedback Survey

We are carrying out a survey to gather feedback from parents/carers on our current Short Breaks services.

Short Breaks are opportunities for children and young people with Special Educational Needs and/or Disabilities (SEND) to enjoy fun activities while giving their families a break from their caring responsibilities.

We want to hear your thoughts on our existing services, any concerns you may have and whether you feel there are any gaps in the Short Breaks offer.

The survey should take approximately 5 minutes to complete, and your input will help us to improve and shape future services.
