Community Wellness Outreach Project
Free Health Checks and support for you! Our mission is to make sure that everyone in West Berkshire has the opportunity to lead a healthy life, focusing on people in vulnerable communities or facing heart-related issues.
If you're an eligible resident between the age of 30 to 74, your GP will contact you to let you know that Solutions4Health will be inviting you for a free NHS Health Check. You will not be eligible for a check if you have had one within the last 5 years, or if you are already diagnosed with, or are being treated for, some cardiovascular health conditions. If you are not eligible for a full check you will be offered a mini-check and advice.
The NHS Health Check is a free check-up of your overall health. It can tell you whether you are at higher risk of getting certain health conditions, such as:
- heart disease
- diabetes
- kidney disease
- stroke
During the check you will discuss how to reduce your risk of these conditions. If you are over 65, you will also be told about symptoms of dementia to look out for.
The health check takes about 30 minutes and includes:
- measuring your height and weight
- measuring your waist
- a blood pressure check
- a cholesterol test
- a blood sugar test
The specialist staff from Solutions4Health will also talk to you about your mental health and any issues impacting on your wellbeing. They can help support you with many things including stopping smoking, losing weight, eating healthier and getting more exercise.
After the health check your results will be recorded and, if you would like, shared with your GP to update your health record. Your age, gender and ethnicity will also be recorded. If you aren't registered with a GP, the team will be able to help you do that.

Hosting an outreach session
If your organisation or group host an outreach session for the benefit of your community or members, or if you are you holding an event that the mobile clinic could attend, let Solutions4Health know. The partners
Solutions4Health will be using specialist trained staff to provide the NHS Health Check and tailored advice and referrals to other support services. Solutions4Health are coordinating the outreach programme with partners across West Berkshire, as well as offering a social prescribing service focused on cardiovascular health. West Berkshire Council are the Commissioners and are providing governance and project support.
GP surgeries and the Primary Care Alliance are providing follow-on clinical support for those that need it.
