Refer a Young Carer

This form is to make a referral for a Young Carer (up to the age of 18 years old) who resides in West Berkshire and has a caring role for a member of their family (parent/sibling/grandparent for example).

The care the person requires may be as a result of poor physical health, poor mental health or substance misuse issues.

The referrer should have the agreement of the parent or guardian prior to submitting this form.
