Complete a Designated Teacher's PEP Submission

This form is for designated teachers to submit information in advance of a Personal Education Planning meeting for a child in care.

Information will be recorded and shared with the pupil and their carers at the Personal Education Planning meeting.

Please use the second person throughout, as if you were speaking directly with the child, e.g. James, you have spent the last 2 weeks learning number bonds from 1 to 20.

We would encourage you to focus on the learning that has taken place and been secured, the next steps and what the pupil and their carers/parents might do to support learning at home. It would be helpful to confirm progress with regards to literacy and numeracy.
